A Note from Continuum's Founder
After graduating from the University of Miami, I spent the first four years of my career as a paralegal (Greenberg Traurig, Kaye Scholer, Manatt, Phelps & Phillips), and then spent the next decade in hedge fund operations, regulatory compliance, and data governance at global financial institutions including Bridgewater Associates, OppenheimerFunds, and BNY Mellon. That may sound like an unusual background for a rehabilitation clinic owner, but it's the reason Continuum is Florida's first AAAHC accredited personal injury-focused wellness center.
Across my career, one lesson held up everywhere: systems matter most when the margin for error is zero. In regulated environments, the gap between a clean process and a sloppy one carries real consequences.
Running complex regulatory data remediation projects taught me what airtight compliance looks like. Derivatives trading operations taught me what documentation that holds up looks like. And building an end-to-end counterparty liquidity workflow at the largest hedge fund in the world taught me how to design systems where nothing falls through the cracks. That same obsession with operational excellence is engineered into every part of Continuum—and it's what sets us apart.
I grew up in a health-focused household that shaped my lifelong appreciation for wellness and patient care. My father is an integrative physician of more than 45 years (now seeing patients at Continuum!), and my mother is a dedicated yoga therapist. Together, they instilled in me the importance of compassionate, holistic healing.
After witnessing my mother’s challenging recovery from an accident, I was inspired to help others on their own healing journeys and saw better ways to do it, beginning with well-designed processes, obsessive documentation, and a world-class care team that communicates with each other between your visits. Dr. Tad DeWald, M.D.
Medical Director
Interventional Pain & Regenerative Medicine Specialist
Dr. Tad DeWald is double Board Certified physician in Sports Medicine and Physical Medicine & Rehabilitation (PM&R), and a leader in non-surgical orthopedic care, with 14+ years of experience in physical medicine.
He earned his medical degree from St. George’s University, completed his PM&R residency at Wayne State University (serving as Chief Resident), and went on to a Sports Medicine fellowship at the renowned Andrews Sports Medicine and Orthopaedic Center in Birmingham, Alabama. After fellowship, he joined Tucson Orthopaedic Institute, where he helped evolve an almost exclusively surgical practice into an international destination for regenerative medicine.
Dr. DeWald is thrilled to serve as Medical Director at Continuum Injury & Wellness Center because the clinic’s integrated model aligns with his commitment to coordinated, evidence-based care, combining image-guided interventions with a clear rehabilitation pathway to help patients recover, restore function, and avoid unnecessary surgery. A member of the Orthobiologic Ethics Consortium, the Interventional Orthobiologics Foundation, and The Orthobiologic Institute Networks, he is dedicated to advancing care with science, integrity, and innovation.

Dr. Amir Mahajer, D.O.
Physical Medicine & Rehabilitation and Sports Medicine
Dr. Amir Mahajer is double Board-Certified in Physical Medicine and Rehabilitation and Sports Medicine, fellowship-trained in Interventional Pain and Sports Medicine at the Icahn School of Medicine at Mount Sinai Hospital.
He is the Founding Physiatrist of Osso Health in South Florida, with a research focus in regenerative and biologic therapies. He serves as Past President of the American Osteopathic College of Physical Medicine and Rehabilitation and as Assistant Professor of Neuroscience at Florida International University Herbert Wertheim College of Medicine. He holds medical licenses in Florida, New York, and California.
A published author and book chapter contributor, his work appears in peer-reviewed journals and texts from Oxford University Press, Human Kinetics, and Springer. He has been featured in Vogue, US News & World Report, PBS, and Healio, and has been recognized as a Top Physiatrist and Top Doctor in Florida and New York, a New York Times Rising Star, and one of America's Best Doctors.
At Continuum, Dr. Mahajer focuses on administering cutting edge interventional pain procedures such as Radiofrequency Ablation.

Dr. Kristal Perez, D.C.
Doctor of Chiropractic
Dr. Kristal Perez is a bilingual chiropractor who delivers precise, evidence‑based care for spine, joint, and soft‑tissue conditions. Trained at Palmer College of Chiropractic–Florida, she blends targeted adjustments (including Diversified, Activator and Thompson techniques) with soft‑tissue therapy, rehab exercises, and clear patient education to relieve pain, restore function, and prevent recurrence. With extensive experience in both clinical care and clinic leadership, her approach is compassionate, communicative, and tailored to each patient’s goals.
At Continuum, Dr. Perez treats acute injuries, chronic neck and back pain, and auto accident cases. She coordinates diagnostic imaging and specialist referrals and is well‑versed in EMS and ultrasound modalities, as well as Personal Injury Protection (PIP) documentation. Fluent in English and Spanish, she helps patients feel informed and supported from first visit to full recovery.

Dr. Robert Kornfeld, DPM
Functional & Regenerative Medicine for Foot & Ankle Pain
Dr. Robert Kornfeld is a functional and regenerative podiatrist, specializing in chronic foot and ankle pain, sports medicine and traumatic foot and ankle injuries. With 43 years of clinical experience in NYC and Long Island, NY, he was the first podiatrist in the U.S to train in and practice both functional and regenerative medicine and has been at the forefront of these fields for 38 years. He's now also seeing patients at Continuum in Fort Lauderdale, FL.
Dr. Kornfeld is the Founder of The Institute for Functional Podiatric Medicine, an institute that trains podiatrists in both functional medicine and regenerative medicine. He has lectured extensively around the country and has been published in many medical journals and publications. He is a member of a newly formed international group of regenerative medicine specialists who share knowledge, experience and research to improve the outcomes of regenerative medicine protocols.
At Continuum, his primary focus is on the non-surgical rehabilitation of the foot and ankle and improving the health of his patients by addressing the underlying mechanisms that prevent healing.
